The Edison Public Library will serve as the host site for a six-week program taking place on Tuesdays throughout October and November that is designed to help you take control of your health.
The Chronic Disease Self-Management Program, developed by Stanford University, is open to those 18 and older who suffer from such chronic conditions as arthritis, heart disease, diabetes, and more. Over the course of six sessions, trained facilitators from the Middlesex County Office of Health Services and Hackensack Meridian Health will help participants with chronic conditions learn how to manage and improve their own health while reducing healthcare costs.
The Chronic Disease Self-Management Program will take place from 1:00pm to 3:30pm at the North Edison Branch on Tuesday, October 8, 15, 22, 29, Wednesday, November 6, and Tuesday, November 12. Participation is limited to 15 and registration is required.
